Although I’ve needed physical therapy for various conditions, knee replacements, etc., the upside was working with Reform Physical Therapy at a location near my home. Not only did the therapy improve my mobility and strength but I experienced a warm and congenial environment there. Upon entry, Nancy greets clients like friends and is efficient and knowledgeable about all the insurance and other questions. The bonus is working with Alex for the third time! He makes the time feel so productive and encourages me to work harder at home as well. A fun aside is trading ideas for recipes and places to eat! I give this place only five stars because there isn’t a higher rating available.

My first physical therapy! I was impressed with Andrew G as he was responsive, listened to my questions and concerns, gave me clear exercises which I did during my first meeting and he gave me a print out of all I should do before my next appointment.
I will have a new physical therapist with my next appointment as Andrew is moving on.
Reform Physical Therapy is wonderful - convenient, plenty of parking, they do not keep you waiting and the receptionist, Nancy, is DA BOMB!

Delaney is amazing....Starting my third bout of PT with her/Reform over last 5 years.
First Issue-Delaney successfully helped me to overcome severe elbow tendinitis (with dry needling and strength work). Issue has not returned in 3 years but I also continue to work on strengthening the elbows during weight training days.
Second Issue-Delaney helped to identify and correct multiple muscular imbalances in my shoulder that were leading to pain/clicking and inability to do any pressing movements without extensive pain. Function has improved dramatically (again with dry needling and strength work).
Currently, just starting working on post operative PT for hip arthroplasty. Looking forward to progressing through the pipeline. Thanks Delaney.

DISCLAIMER:
They aren't miracle workers. To be successful you have to be willing to put in the work at home. If you are honest with them during assessment, listen to what they have to say, and legitimately put in the effort on your exercises, you will do well.

I would like to begin by recognizing Nancy, who manages the Freeport office with exceptional skill and grace. Although she sits modestly at the front desk handling scheduling, insurance, and co‑pays, she is truly the organizational backbone of this outstanding Reform Physical Therapy team. Her mastery of scheduling consistently ensures that my appointments fit seamlessly into my day. Nancy provides a warm, welcoming first impression—something that means more to a patient walking into a clinic than many people realize. You are very fortunate to have her.

I have also had the pleasure of working with Delaney Gaetano and Andrew Gendreau during my recovery from a rotator cuff repair. Both have been consistently caring, knowledgeable, and thorough. They take the time to explain each exercise and movement, helping me understand how they contribute to my healing. Their positive energy is evident the moment I walk into the gym—they greet me with a smile, offer encouragement, and even share a few recipes along the way. I genuinely look forward to each session.

Although Andrew will be moving on soon, it is clear that this has been a wonderful environment for him to learn and grow, alongside Delaney and Alex. The quality of care here is exceptional, and the people delivering it are even better. Thank you for hiring such dedicated, compassionate professionals.

Sincerely,
Thomasa (Tommie) Whisler

Had a great experience at Reform PT in Freeport after knee replacement surgery. Alex is patient and knowledgeable and carries on great conversation. Delaney worked with me one day with 2 student observers and I was impressed by her thoroughness since she hadn’t worked with me before. Nancy at the front desk is a marvel of friendly efficiency.
I’m doing really well now continuing the exercises I was taught. Thank you all!—Jane K.

Reform physical therapy is conveniently located right off of exit 22 in the Freeport Medical Center. I called following surgery and spoke with Nancy who understood my needs and got me scheduled quickly. I found the atmosphere to be professional and quite convivial. I worked with Alex who was knowledgeable and had a great approach to patient-centered care. My entire experience healing with their therapy as a post op patient has been very positive overall. I recommend this practice without hesitation.
