Tendon injuries can make even simple movements uncomfortable. Understanding how tendon injury physical therapy can help is important for recovery. Walking up the stairs, reaching overhead, gripping something, running, lifting weights, or getting through a workday may suddenly become harder than it used to be. Sometimes tendon pain begins after one specific injury. Other times, it builds gradually after weeks or months of repeated activity.

Tendons are strong tissues that connect muscles to bones. They play an important role in transferring force from your muscles so your joints can move. When a tendon becomes irritated, overloaded, or torn, pain and weakness can interfere with everyday movement. Tendon injury physical therapy can help you gradually rebuild strength and prepare the tendon for the activities you want to return to. What Is a Tendon Injury?

A tendon is a strong band of connective tissue that connects a muscle to a bone. When your muscle contracts, the tendon helps transfer that force to the bone and creates movement. Tendons throughout your body handle significant amounts of force every day. They help you walk, run, jump, lift, throw, grip, climb, and perform countless other movements. Problems can develop when a tendon is suddenly injured or repeatedly exposed to more stress than it can currently tolerate. Tendon injuries range from temporary irritation to long-term tendon changes and partial or complete tears.

Tendon Injury vs. Muscle Injury

Tendons and muscles work closely together, which can make it difficult to tell where pain is coming from. A muscle strain involves injury to the muscle itself or the area where muscle transitions into tendon. A tendon injury primarily affects the tissue connecting the muscle to bone. Both can cause pain and weakness. The location of your symptoms, how the injury occurred, and how the area responds to strength and movement testing can help your physical therapist determine which tissue may be involved.

What Does a Tendon Injury Feel Like?

Tendon symptoms vary depending on the tendon involved and the type of injury. Pain may be sharp after a sudden injury or develop as a more gradual ache. You might notice pain when you first begin moving, during a specific exercise, after activity, or the following morning. The area may feel stiff or tender. Some people notice weakness or difficulty producing force through the affected arm or leg. Symptoms can also improve once you “warm up” and then return later after activity. A complete tendon tear may cause much more significant weakness or loss of normal function.

What Causes Tendon Injuries?

There is not always one single cause. Some tendon injuries happen suddenly during sports, lifting, falls, or other forceful movements. Others develop gradually when the amount of activity placed on the tendon increases faster than the tissue can adapt. For example, a runner may suddenly increase mileage or add hills. Someone may begin playing pickleball several times each week after being relatively inactive. A homeowner might spend an entire weekend completing yardwork. A worker may perform the same lifting or gripping motion hundreds of times. The activity itself is not necessarily the problem. The tendon may simply need more time and progressive training to tolerate the new demand.

Acute Tendon Injuries

An acute tendon injury happens suddenly. You may feel a sharp pain during running, jumping, lifting, throwing, or another forceful movement. Some people describe feeling or hearing a pop. The area may quickly become painful, swollen, or weak. More significant injuries can make it difficult to use the affected body part normally. A sudden loss of strength after an injury should be evaluated because it can be a sign of a more serious tendon tear.

Overuse Tendon Injuries

Many tendon problems develop gradually rather than during one dramatic event. Repeated activity places stress on tendons. Usually, your body adapts to that stress. Problems can develop when activity increases too quickly or the tendon does not have enough time to adapt between repeated demands. Changes in exercise volume, work demands, sports participation, equipment, terrain, or training intensity can all contribute. Physical therapy can help determine what the tendon currently tolerates and gradually build its capacity.

What Is Tendinitis?

Tendinitis refers to inflammation involving a tendon. The term is often used for tendon pain, particularly when symptoms are relatively new. You may experience pain, tenderness, and difficulty performing movements that load the affected tendon. However, not all ongoing tendon pain is primarily caused by inflammation. That distinction becomes important when symptoms have lasted for a longer period.

What Is Tendinopathy?

Tendinopathy is a broader term used to describe painful tendon conditions. It may include tendinitis as well as longer-term changes within the tendon. Tendon problems can occur in many areas of the body, including the Achilles tendon, patellar tendon, rotator cuff, elbow tendons, hamstring tendons, and tendons surrounding the hip, wrist, and thumb. The important part for rehabilitation is understanding what movements load the tendon and how much stress it can currently tolerate.

Tendinitis vs. Tendinosis

These terms sound similar but describe different tendon changes. Tendinitis generally refers to inflammation within or around a tendon. Tendinosis refers to longer-term changes in the tendon tissue, including changes to the collagen that makes up the tendon. This is one reason persistent tendon pain should not automatically be treated as an inflammatory problem. Your rehabilitation plan should be based on your specific symptoms and how the tendon responds to movement and loading.

Achilles Tendon Injuries

The Achilles tendon connects the calf muscles to the heel. It handles substantial forces during walking, running, jumping, and pushing off the ground. Achilles tendon problems may develop gradually or occur suddenly. Someone with Achilles tendinopathy may notice stiffness when taking their first steps in the morning, discomfort during running, or pain after activity. A sudden Achilles rupture is different. People sometimes describe feeling as though they were kicked in the back of the ankle, followed by difficulty pushing off through the foot. Suspected Achilles ruptures require prompt medical evaluation.

Patellar Tendon Injuries

The patellar tendon is located at the front of the knee and connects the kneecap to the shinbone. It plays an important role during jumping, running, squatting, and climbing stairs. Patellar tendon problems are sometimes called jumper’s knee because they frequently occur in sports involving repeated jumping. However, you do not need to be an athlete to develop patellar tendon pain. Rehabilitation typically focuses on gradually rebuilding the tendon and surrounding muscles’ ability to tolerate force.

Quadriceps Tendon Injuries

The quadriceps tendon connects the large muscles at the front of the thigh to the kneecap. Problems in this area can cause pain above the knee and difficulty with activities such as squatting, climbing stairs, running, or jumping. A complete quadriceps tendon tear can significantly affect the ability to straighten the knee and requires medical evaluation. Less severe tendon problems may respond well to progressive rehabilitation.

Rotator Cuff Tendon Injuries

The rotator cuff is a group of muscles and tendons surrounding the shoulder. These tissues help control and stabilize the shoulder during movement. Rotator cuff tendon problems can cause pain when reaching overhead, lifting, throwing, reaching behind the body, or sleeping on the affected side. Some people are told they have rotator cuff tendinitis, tendinopathy, or a partial tendon tear. Treatment depends on your symptoms, strength, function, and the severity of the injury. Not every rotator cuff tendon problem requires surgery.

Tennis Elbow

Tennis elbow involves tendons around the outside of the elbow. Despite the name, you do not need to play tennis to develop it. Gripping, lifting, carrying, typing, tool use, weight training, and repetitive hand or wrist activity may contribute. Pain commonly occurs near the outside of the elbow and may travel into the forearm. Rehabilitation can help gradually strengthen the muscles and tendons involved in gripping and wrist movement.

Golfer’s Elbow

Golfer’s elbow affects tendons along the inside of the elbow. Like tennis elbow, it is not limited to athletes. Repeated gripping, lifting, throwing, or wrist movements can place stress on the area. Treatment may involve temporarily adjusting irritating activities while gradually rebuilding forearm and grip strength.

Hamstring Tendon Injuries

The hamstring tendons connect the muscles along the back of your thigh to bones around the pelvis and knee. Some people develop pain near the sitting bone at the top of the hamstring. This can make prolonged sitting, running, lunging, or bending uncomfortable. Other hamstring tendon injuries occur closer to the knee. Rehabilitation should consider where the tendon is injured and which activities place the greatest demand on it.

Hip Tendon Injuries

Several tendons surrounding the hip can become painful. Gluteal tendons along the outside of the hip are a common example. People may experience pain while walking, climbing stairs, standing on one leg, or lying on the affected side. Hip tendon pain can sometimes be mistaken for bursitis because the structures are located close together. Your therapist can evaluate strength, mobility, and movement to determine which factors may be contributing.

Foot and Ankle Tendon Injuries

Several tendons cross the foot and ankle. They help support the foot, control ankle movement, and create force when walking and running. Tendon problems in this region may cause pain along the inside or outside of the ankle, around the heel, or through the foot. Physical therapy may address foot and ankle strength, calf strength, balance, mobility, and tolerance for walking or running.

Hand and Wrist Tendon Injuries

The tendons in your hands and wrists allow you to bend and straighten your fingers, move your thumb, and control the wrist. Overuse may cause tendon irritation, while cuts, falls, sports injuries, or forceful movements can cause more significant damage. A complete tendon injury can make it difficult or impossible to perform a specific finger or wrist movement. Deep cuts and sudden loss of movement should receive prompt medical evaluation.

Tendon Injuries From Running

Running places repeated loads through the tendons of the hips, knees, ankles, and feet. Tendon pain sometimes appears after someone suddenly increases mileage, speed, hills, or the number of running days each week. Stopping all activity is not always the long-term answer. The goal is often to temporarily adjust the amount of stress on the tendon and then progressively rebuild its ability to handle running. Your physical therapist can help determine an appropriate starting point.

Tendon Injuries From Sports

Sports often require tendons to manage large amounts of force quickly. Jumping, sprinting, cutting, throwing, swinging, and changing direction all place different demands on the body. Rehabilitation should eventually prepare the tendon for those same demands. An athlete who wants to return to basketball needs more than basic strength. A tennis player needs to tolerate repeated gripping and swinging. A runner needs to handle repeated impact. Your treatment plan should reflect your sport.

Tendon Injuries From Work

Tendon problems can also develop from work. Repeated lifting, gripping, reaching, carrying, climbing, or using tools can place significant demands on tendons. If symptoms interfere with your job, rehabilitation should consider the movements you perform throughout your actual workday. Your therapist can gradually progress exercises toward those demands rather than stopping treatment as soon as basic movement becomes comfortable.

Why Does Tendon Pain Keep Coming Back?

Tendon symptoms can improve when activity decreases and return when the same activity resumes. This does not necessarily mean the tendon is permanently damaged. It may mean the tendon has not yet rebuilt enough capacity for the amount of stress being placed on it. For example, resting from running may decrease Achilles pain. If you immediately return to your previous mileage, symptoms may return because the tendon has not been gradually prepared for that workload. A progressive strengthening and return-to-activity plan can help close that gap.

Is Rest Enough for a Tendon Injury?

Short-term activity modification may help calm an irritated tendon. Complete long-term rest, however, may leave the tendon and surrounding muscles less prepared for activity. Tendons respond to appropriate loading. Physical therapy helps find the balance between doing too much and doing too little. You may temporarily reduce the activity causing symptoms while continuing other exercises and gradually strengthening the affected area.

Should You Stretch a Painful Tendon?

Stretching is not automatically the answer to tendon pain. Some tendon problems may tolerate stretching well. Others can become more irritated when repeatedly placed into certain positions. A feeling of tightness also does not necessarily mean the tendon needs aggressive stretching. Your physical therapist can determine whether mobility work is appropriate and how it should fit into your overall treatment plan.

Should You Massage a Tendon Injury?

Hands-on treatment may help some people manage pain or improve movement in the surrounding area. However, massage alone does not rebuild a tendon’s ability to tolerate force. Exercise and progressive loading are often important parts of tendon rehabilitation. At Reform, hands-on treatment can be combined with active rehabilitation when it supports your overall goals.

Can Physical Therapy Help Tendon Injuries?

Yes. Tendon injury physical therapy can help many people recover from tendon pain and injury. Treatment usually begins by identifying which activities are irritating the tendon and how much load it currently tolerates. Your therapist can then introduce exercises designed to gradually increase strength. As your capacity improves, resistance and activity demands can progress. Eventually, treatment can become more specific to walking, running, lifting, work, or sports. Physical therapy is commonly used as part of care for tendinopathy and other tendon conditions.

Strengthening Tendons

Strengthening is an important part of many tendon rehabilitation programs. The goal is not to overload the tendon immediately. Instead, exercises begin at a level the tissue can manage and gradually become more challenging. Depending on the injury, this may involve isometric exercises, controlled resistance training, eccentric exercises, or other forms of progressive strengthening. Over time, heavier or faster movements may be introduced when appropriate.

What Is Eccentric Strengthening?

An eccentric muscle contraction occurs when a muscle produces force while lengthening. For example, slowly lowering your heel during a calf exercise involves eccentric work from the calf and Achilles tendon. Eccentric exercises have historically been used frequently for tendon rehabilitation. However, tendon rehabilitation does not need to consist entirely of eccentric exercise. The right strengthening program depends on the tendon, stage of recovery, symptoms, and activities you want to return to.

Progressive Loading for Tendon Injuries

One of the major goals of tendon rehabilitation is improving how much load the tendon can tolerate. That process needs to be gradual. If exercises are always extremely easy, the tendon may not receive enough challenge to adapt. If the workload increases too quickly, symptoms may become more irritated. Your therapist can adjust resistance, repetitions, speed, range of motion, and frequency based on how you respond.

Is Some Pain During Tendon Exercise Normal?

Not every sensation during tendon rehabilitation means you are causing damage. Depending on the condition, some discomfort during or after exercise may be acceptable. The important part is how much discomfort occurs and how the tendon responds afterward. Your physical therapist can give you specific guidelines for your injury rather than asking you to guess whether you are doing too much.

Returning to Running After a Tendon Injury

Returning to running should usually happen gradually. Your therapist may first evaluate strength, walking tolerance, single-leg control, hopping, and other movements. A return-to-running program may begin with shorter distances or walk-run intervals. Mileage and speed can increase as the tendon demonstrates that it can tolerate more activity.

Returning to the Gym

You may not need to completely stop strength training because of a tendon injury. Instead, exercises can often be modified. Your therapist may adjust weight, range of motion, exercise selection, repetitions, or training frequency. As symptoms and strength improve, you can gradually work toward your previous training level.

Returning to Sports

Sports rehabilitation should eventually look more like the activity you are preparing for. A jumping athlete may need progressively faster and more explosive exercises. A throwing athlete may need shoulder, arm, and trunk strengthening. A runner needs repeated loading. A racquet-sport athlete may need grip strength, rotation, and quick changes in direction. Your therapist can help bridge the gap between basic rehabilitation and the demands of your sport.

How Long Do Tendon Injuries Take to Heal?

There is no single recovery timeline for every tendon injury. A mild, newer problem may improve faster than a long-standing tendinopathy or significant tendon tear. The tendon involved also matters. Your activity level, general health, previous injuries, severity of tissue damage, and the demands you want to return to can all influence recovery. Tendon problems can take time, particularly when symptoms have been present for months. Progress should be measured by more than pain alone. Strength, function, activity tolerance, and confidence also matter.

Do Tendon Tears Require Surgery?

Not every tendon tear requires surgery. Treatment depends on the tendon involved, how much of the tendon is torn, your level of weakness, your age, your activity goals, and other factors. Some partial tendon tears may be managed without surgery. Complete tears of certain tendons are more likely to require surgical evaluation. Your orthopedic provider can help determine the appropriate treatment.

Physical Therapy After Tendon Surgery

Some significant tendon injuries require surgical repair. Physical therapy after tendon surgery follows a carefully structured progression because the repaired tissue needs time to heal. Early rehabilitation may focus on protecting the repair while maintaining appropriate movement. Strengthening is gradually introduced according to the surgeon’s protocol. Later treatment can focus on restoring strength and preparing you for everyday activity, work, exercise, or sports. Reform includes tendon repair among the conditions that may benefit from pre- and post-surgical rehabilitation.

When Is Imaging Needed?

Not every case of tendon pain requires an MRI. A healthcare provider may be able to identify many tendon problems through your history and physical examination. Imaging may be recommended when a significant tear is suspected, symptoms are unusual, or the diagnosis remains unclear. Ultrasound and MRI can provide additional information about tendon tissue when needed.

When Should You Seek Medical Attention?

Seek medical evaluation if you experience a sudden pop followed by significant weakness, cannot use the affected arm or leg normally, notice a visible change in the muscle or tendon, or develop severe swelling or bruising. A deep cut over a tendon should also receive prompt medical attention. Sudden inability to push off through the foot after an Achilles injury, inability to straighten the knee after a significant injury, or sudden loss of shoulder or hand function should not simply be treated as routine tendon soreness. If symptoms continue worsening or are not improving as expected, additional evaluation may also be appropriate.

What Happens During a Tendon Injury Physical Therapy Evaluation?

Your therapist will begin by learning about your symptoms. We will want to know where you feel pain, when it started, what activities make it worse, and what you want to return to doing. Your evaluation may include mobility, strength, tenderness, functional movement, balance, and specific tests for the injured area. A runner may eventually have running-related testing. Someone with an elbow tendon problem may have grip strength assessed. A patient with shoulder symptoms may complete reaching and resistance testing. The goal is to understand how the tendon and surrounding area are functioning, not simply identify where it hurts.
