Tendon pain can turn movements you normally do without thinking into frustrating daily challenges. Your shoulder may hurt when you reach into a cabinet. Your elbow may bother you when you pick up a coffee mug. Maybe your knee hurts when you climb stairs, or your Achilles feels stiff every morning when you first get out of bed. These symptoms can sometimes be related to tendonitis. Tendonitis physical therapy is often a key part of addressing these issues. Tendons connect muscles to bones and help transfer force whenever you move. When a tendon becomes irritated or inflamed, it can cause pain, tenderness, stiffness, and difficulty using the affected area.
Tendonitis physical therapy can help address the movements that are bothering the tendon while gradually rebuilding strength and tolerance for activity. What Is Tendonitis?
Tendonitis is irritation or inflammation of a tendon. Tendons are strong bands of connective tissue that attach muscles to bones. They allow the force created by your muscles to produce movement at your joints. Because tendons are involved in nearly every movement you make, they regularly handle significant amounts of force. Symptoms can develop when a tendon becomes irritated after a sudden increase in activity, repetitive movement, or an injury. Tendonitis can occur throughout the body, but it is especially common around the shoulders, elbows, knees, ankles, and feet.
What Does Tendonitis Feel Like?
Tendonitis often causes pain near a joint where the tendon attaches to bone. The area may feel tender when you touch it or uncomfortable when you use the muscle connected to the tendon. Some people experience stiffness after sitting or resting. Others notice symptoms first thing in the morning. Pain may improve after you begin moving and then return later during or after activity. As symptoms become more noticeable, you may also begin changing how you move to avoid the painful area.
What Causes Tendonitis?
Tendonitis can develop for several reasons. Sometimes symptoms appear after one specific injury. More commonly, pain develops after repeated activity or a change in how much stress is being placed on the tendon. You may have started a new exercise program, increased your running mileage, played more pickleball than usual, completed a weekend of yardwork, or spent several long days performing repetitive tasks at work. The activity itself is not necessarily harmful. The tendon may simply be experiencing more demand than it is currently prepared to handle.
Can Tendonitis Develop Suddenly?
Yes. Although tendon pain commonly builds gradually, symptoms can sometimes appear after a sudden increase in activity. Maybe you haven’t played tennis in months and spend an entire weekend on the court. Perhaps you start a new strength program and dramatically increase your training volume. The tendon may respond to that unfamiliar workload with pain and irritation. This does not necessarily mean you need to permanently stop the activity. Your body may need a more gradual progression.
Tendonitis vs. Tendinopathy
Tendonitis and tendinopathy are often used interchangeably, but they are not exactly the same. This specifically refers to inflammation involving a tendon. Tendinopathy is a broader term used for tendon pain and changes that can develop within the tendon. When tendon symptoms continue for a longer period, inflammation may no longer be the main issue. This distinction is important because long-lasting tendon pain often requires more than simply trying to reduce inflammation. Progressive strengthening and activity management may become important parts of recovery.
Tendonitis vs. Tendinosis
Tendinosis generally describes longer-term changes within a tendon rather than an acute inflammatory response. Someone who has experienced tendon pain for several months may still call the problem tendonitis even if the condition behaves more like tendinopathy or tendinosis. You do not need to figure out the terminology on your own. A physical therapy evaluation can help determine how your symptoms behave and what type of rehabilitation may be appropriate.
Tendonitis vs. a Tendon Tear
Tendonitis does not automatically mean the tendon is torn. Tendon irritation may cause pain while the tendon remains intact. A tendon tear involves actual disruption of tendon fibers. Partial tears involve some of the tendon fibers, while a complete rupture involves a full tear. Significant weakness, sudden loss of function, a popping sensation during an injury, or visible changes around the muscle or tendon may indicate a more serious injury and should be medically evaluated.
Shoulder Tendonitis
The shoulder contains several tendons that help control movement. Rotator cuff tendonitis is one of the more common causes of shoulder pain. Symptoms may occur when reaching overhead, lifting, throwing, reaching behind your back, or sleeping on the affected shoulder. You may also notice a painful arc when raising your arm. Shoulder tendon pain does not automatically mean you have a major tear or need surgery. Physical therapy can evaluate shoulder strength, mobility, and movement to determine an appropriate treatment plan.
Rotator Cuff Tendonitis
The rotator cuff consists of muscles and tendons surrounding the shoulder joint. These tissues help keep the shoulder stable while allowing your arm to move. Rotator cuff tendonitis can develop after repeated overhead activity, lifting, throwing, swimming, or changes in exercise. Treatment may include temporarily modifying irritating movements while improving shoulder and upper-back mobility and progressively strengthening the rotator cuff.
Biceps Tendonitis
The biceps muscle has tendons that connect around the shoulder and elbow. Pain involving the upper biceps tendon is often felt near the front of the shoulder. Lifting, reaching, throwing, or performing repeated overhead movements may increase symptoms. Because several shoulder conditions can cause pain in a similar area, an evaluation can help determine whether the biceps tendon or another structure may be contributing.
Tennis Elbow
Tennis elbow is a common tendon condition affecting the outside of the elbow. Despite its name, many people with tennis elbow have never played tennis. Repeated gripping, lifting, carrying, tool use, computer work, weight training, and racquet sports can all place stress on the tendons around the elbow. Symptoms may make everyday tasks such as opening a jar, carrying groceries, or holding a coffee cup uncomfortable. Physical therapy can gradually rebuild wrist, forearm, and grip strength.
Golfer’s Elbow
Golfer’s elbow affects tendons along the inside of the elbow. It can develop from repeated gripping, lifting, throwing, swinging, or wrist movements. Golfers can certainly experience it, but so can workers, weightlifters, racquet-sport athletes, and people performing repetitive tasks. Treatment is based on gradually improving the ability of the involved muscles and tendons to tolerate those movements.
Patellar Tendonitis
The patellar tendon is located at the front of the knee and connects the kneecap to the shinbone. Patellar tendonitis is sometimes called jumper’s knee. Running, jumping, squatting, and other activities that repeatedly load the knee can irritate the area. Pain is often felt just below the kneecap. Rehabilitation may include strengthening the quadriceps, hips, and surrounding muscles while gradually rebuilding tolerance for jumping, running, or other demanding activities.
Quadriceps Tendonitis
The quadriceps tendon connects the large muscles at the front of your thigh to the top of the kneecap. Pain is typically felt just above the kneecap. Squatting, stairs, running, jumping, or repeated knee bending may become uncomfortable. A progressive strengthening program can help improve how the knee and quadriceps handle these activities.
Achilles Tendonitis
The Achilles tendon connects your calf muscles to the heel. It handles significant amounts of force during walking, running, jumping, and pushing off through the foot. Achilles tendonitis may cause pain or stiffness near the back of the ankle. Morning stiffness is common. Your first few steps after getting out of bed may be uncomfortable before the area begins loosening up. Running may initially feel okay and then become painful during or after activity. Physical therapy can help gradually rebuild calf and Achilles strength while guiding your return to walking, running, and sports.
Tendonitis in the Foot and Ankle
The Achilles is not the only tendon that can become painful around the foot and ankle. Several tendons help support the foot and control ankle movement. Symptoms may develop along the inside or outside of the ankle or through portions of the foot. Walking, running, standing, and uneven surfaces may become uncomfortable. Treatment may include foot and ankle strengthening, calf strengthening, balance exercises, mobility work, and gradual progression of weight-bearing activities.
Hip Tendonitis
Tendons surrounding the hip can also become irritated. Pain may occur along the outside, front, or back of the hip depending on which tendon is involved. Gluteal tendon problems commonly cause pain along the outside of the hip. Walking, stairs, standing on one leg, or lying on the affected side may increase symptoms. Because hip tendon pain can sometimes feel similar to other hip conditions, an evaluation can help identify which movements and tissues may be contributing.
Hamstring Tendonitis
Hamstring tendon pain often occurs near the upper hamstring where it attaches close to the sitting bone. Running, lunging, bending, or sitting for long periods may aggravate symptoms. Stretching the hamstring aggressively is not automatically the best solution. In some cases, repeated stretching can place additional stress on an already sensitive tendon. Rehabilitation often focuses on gradually rebuilding hamstring strength and tolerance for activity.
Wrist and Hand Tendonitis
Your hands and wrists perform hundreds of small movements throughout the day. Typing, gripping, lifting, carrying, tool use, sports, and repetitive hand movements can contribute to tendon irritation. Pain may occur near the wrist, thumb, or hand depending on which tendon is involved. Physical therapy can evaluate strength, mobility, and the activities that repeatedly place stress on the area.
Tendonitis From Running
Running requires tendons throughout the legs to repeatedly store and release energy. Your Achilles tendon, patellar tendon, hamstring tendons, and several tendons around the hip, foot, and ankle all contribute. Tendon symptoms may appear after a sudden increase in mileage, pace, hills, or frequency. Instead of simply telling you to stop running indefinitely, rehabilitation can help determine what amount of running the tendon currently tolerates. You can then gradually build from there.
Tendonitis From Pickleball and Racquet Sports
Pickleball, tennis, and other racquet sports involve repeated gripping, swinging, quick starts, stops, and changes of direction. This can place demands on tendons throughout the elbows, shoulders, knees, and ankles. If you recently started playing more often, your overall workload may have increased faster than your body was prepared for. Treatment can address the painful area while also preparing you for the movements required by your sport.
Tendonitis From Weightlifting
Strength training is generally beneficial for muscles, tendons, joints, and overall health. However, tendon symptoms can appear when training volume or intensity changes too quickly. Maybe you dramatically increased the weight you are lifting. Perhaps you added more training days or started performing a new exercise. Rather than automatically avoiding strength training, your program can often be modified while the tendon gradually becomes stronger.
Tendonitis From Work
Repetitive tasks at work can place substantial demands on tendons. Gripping tools, lifting, carrying, reaching overhead, typing, pushing, pulling, and working in the same position repeatedly may contribute to symptoms. Your rehabilitation should consider your actual job. If you need to lift throughout an eight-hour workday, your tendon eventually needs to tolerate more than a few light exercises in the clinic. Physical therapy can gradually prepare you for those demands.
Why Does Tendonitis Hurt in the Morning?
Morning stiffness is common with several tendon conditions. After being relatively still overnight, the tendon may initially feel stiff or uncomfortable when you begin moving. Symptoms may decrease after several minutes of activity. That does not necessarily mean the tendon is fully recovered. If pain continues returning each morning, the underlying tendon capacity and daily workload may still need to be addressed.
Why Does My Tendon Feel Better After Warming Up?
Some people with tendon pain notice that the first few movements are uncomfortable, but symptoms decrease once they warm up. This can make it tempting to assume the problem is gone. However, symptoms may return later during the activity or afterward. Pay attention to how the tendon responds throughout the entire day and the following morning, not only during the first few minutes of exercise.
Should You Rest Tendonitis?
Temporarily reducing an irritating activity may be helpful. That does not always mean completely resting until all pain disappears. Long periods of inactivity can reduce strength and make returning to your previous activity more difficult. Instead, rehabilitation often involves finding a manageable amount of activity while progressively strengthening the tendon. Your therapist can help determine which activities should temporarily change and which you can safely continue.
Should You Stretch Tendonitis?
It depends. Stretching may help when limited mobility is contributing to your movement problem, but aggressive stretching is not automatically appropriate for every tendon condition. Some tendon positions create compression or additional stress on the painful area. If stretching repeatedly makes symptoms worse, more stretching may not be the solution. Your therapist can determine whether flexibility work belongs in your treatment plan.
Is Massage Good for Tendonitis?
Hands-on treatment may help decrease discomfort or improve mobility in surrounding tissues for some patients. However, massage alone does not improve a tendon’s long-term ability to tolerate force. Active rehabilitation is important. At Reform, manual therapy can be combined with strengthening and movement when hands-on treatment supports your overall rehabilitation goals.
Should I Use Ice or Heat?
Ice and heat may temporarily change how the area feels, but neither is a complete treatment for tendonitis. Some people prefer ice when the area feels irritated after activity. Others prefer warmth when stiffness is the bigger complaint. Either can be used for comfort when appropriate. The long-term goal is to improve the tendon and surrounding muscles’ ability to tolerate movement and activity.
Can Physical Therapy Help Tendonitis?
Yes. Tendonitis physical therapy can help identify which movements are irritating the tendon and progressively improve its ability to tolerate activity. Your treatment may initially involve modifying aggravating activities. Exercise can then be introduced at an appropriate level. As the tendon adapts, resistance, repetitions, range of motion, speed, and functional demands can gradually increase. The goal is to help you return to the activities you care about rather than simply avoiding them.
Strength Training for Tendonitis
Strengthening is an important part of many tendon rehabilitation programs. Tendons respond to mechanical load. The challenge is providing enough load to encourage adaptation without suddenly overwhelming an irritated tendon. Your exercises may begin relatively light. Over time, they can become progressively more challenging. Someone returning to everyday activity may need basic strengthening. An athlete may eventually need heavier resistance, jumping, running, throwing, or faster movements.
Eccentric Exercises for Tendonitis
Eccentric exercise involves producing force while a muscle is lengthening. Slowly lowering your heel during a calf raise is one example. Eccentric exercises have been widely used in tendon rehabilitation, particularly for Achilles and other lower-extremity tendon problems. However, they are not the only type of strengthening that can help. Your program may include several forms of muscle and tendon loading depending on your condition.
Isometric Exercises for Tendon Pain
Isometric exercises involve creating muscle tension without much joint movement. For example, you may push against resistance while holding a position. These exercises can sometimes provide a useful starting point when a tendon is sensitive to larger movements. As symptoms and strength improve, your program can progress toward exercises involving greater ranges of motion and heavier resistance.
Progressive Loading
Progressive loading means gradually increasing what the tendon is asked to handle. That may include more resistance, repetitions, movement, speed, or activity. If the tendon is never challenged, it may not develop the capacity needed for your normal activities. If the workload increases too quickly, symptoms may flare. Finding the appropriate middle ground is an important part of rehabilitation.
Is Pain During Tendonitis Exercise Okay?
Some discomfort during tendon rehabilitation does not automatically mean you are damaging the tendon. The acceptable amount depends on the condition and your individual response. Your therapist may ask you to monitor symptoms during exercise, later that day, and the following morning. That information can help determine whether your current exercise level is appropriate or needs to be adjusted.
Returning to Running With Tendonitis
Running does not always need to be completely eliminated. Depending on your symptoms, you may temporarily decrease mileage, pace, hills, or running frequency. As strength improves, running can gradually increase. Your therapist may use strength testing, single-leg exercises, hopping, and other functional movements to help determine when the tendon is ready for greater impact.
Returning to Sports
Returning to sports requires preparing the tendon for the forces involved in your specific activity. A basketball player needs to jump and land. A tennis or pickleball player needs to grip, swing, and change direction. A runner needs to tolerate repeated impact. A swimmer or throwing athlete needs repeated shoulder movement. Your later rehabilitation should begin to reflect those demands.
How Long Does Tendonitis Take to Heal?
There is no single timeline. A newer tendon problem may improve faster than symptoms that have been present for months. The tendon involved, your activity level, overall health, strength, and daily demands all influence recovery. Tendon rehabilitation often requires patience because building tissue capacity takes time. Instead of judging recovery only by pain, we also look at strength, movement, function, and your ability to perform activities without repeated flare-ups.
Why Does My Tendonitis Keep Coming Back?
A common pattern is to rest until the pain improves and then immediately return to the same amount of activity. The tendon feels better, but it has not necessarily become more prepared for that workload. Symptoms then return. A better approach may involve gradually rebuilding strength while slowly increasing the activity that caused symptoms. This helps prepare the tendon rather than repeatedly moving between complete rest and full activity.
Can Tendonitis Become Chronic?
Yes. Tendon symptoms can sometimes continue for months. At that point, the condition may be more accurately described as tendinopathy rather than acute tendonitis. Long-lasting symptoms do not automatically mean you will always have pain. Your rehabilitation may simply need to focus more heavily on progressive strengthening, workload management, and gradually returning to activity.
Does Tendonitis Require Surgery?
Most cases of tendonitis do not automatically require surgery. Conservative treatment, including physical therapy and activity modification, is often considered first. Surgery may become relevant when there is a significant tendon tear or another structural problem that requires repair. Your healthcare provider can help determine whether additional medical treatment is appropriate.
When Should You See a Doctor?
Seek medical evaluation if you experience a sudden pop followed by significant weakness or loss of function. Major bruising, swelling, visible deformity, or inability to use the affected arm or leg normally can indicate a more serious injury. You should also seek evaluation if symptoms are severe, continue worsening, or do not improve as expected. Tendon pain can sometimes resemble other conditions, so persistent symptoms deserve appropriate assessment.
What Happens During a Tendonitis Physical Therapy Evaluation?
Your physical therapist will begin by learning about your symptoms and goals. We will ask when the pain began, where you feel it, which activities make it worse, and whether your activity level recently changed. Your therapist may then evaluate mobility, strength, tenderness, balance, and functional movement. Testing will depend on the affected area. Shoulder tendonitis may involve reaching and rotator cuff strength testing. Achilles symptoms may involve calf raises, walking, or hopping. Elbow tendonitis may include grip and wrist strength testing. The goal is to understand how the painful area is functioning and what needs to improve.
