Both active movements, when you move your own arm, and passive movement, when someone else moves it for you, are typically restricted.

The condition often develops gradually rather than after one major injury. In some cases, there is no clear reason why it begins. Other people develop frozen shoulder after surgery, an injury, or a period when the arm has not been moving normally. Because symptoms change over time, frozen shoulder treatment should match the current stage of the condition rather than using the same exercises throughout the entire recovery.

Common Frozen Shoulder Symptoms

Pain and stiffness are the main symptoms of frozen shoulder. Early in the condition, pain may be more noticeable than the loss of movement. As time passes, the shoulder may become increasingly difficult to move. Reaching overhead may become painful or impossible. Getting an arm into a coat sleeve, fastening a bra, washing your hair, reaching into a cabinet, or placing a hand behind your back may also become difficult.

Some people notice that the shoulder aches even when they are not moving it. Night pain can make sleeping uncomfortable, especially when lying on the affected side. The stiffness may eventually become more frustrating than the pain itself because it limits so many normal movements.

Loss of Shoulder Movement

Frozen shoulder causes a noticeable loss of motion in several directions. Lifting the arm overhead often becomes difficult. Rotating the arm outward can be especially limited. Reaching behind the back may also become increasingly restricted.

Unlike some shoulder conditions, frozen shoulder usually limits movement even when someone else tries to gently move your arm. This loss of both active and passive motion is an important finding during an evaluation. The amount of stiffness can vary. Some people maintain enough movement for most basic activities, while others develop severe restrictions that affect dressing, bathing, driving, and work.

Shoulder Pain at Night

Night pain is common during the painful stage of frozen shoulder. Finding a comfortable sleeping position may become difficult because pressure against the shoulder increases discomfort. Rolling onto the painful side can wake some people from sleep. Pain may also continue when lying on the opposite side because the affected arm is unsupported. Your therapist may discuss pillow positioning or ways to support the arm while sleeping. These strategies do not cure the condition, but they may make nighttime symptoms easier to manage.

Difficulty Reaching Overhead

Overhead movement requires the shoulder joint and shoulder blade to work together. As the shoulder capsule becomes stiff, reaching into cabinets, washing hair, putting on a shirt, or lifting something overhead may become difficult. People sometimes compensate by leaning their body, lifting the shoulder toward the ear, or using the opposite arm. Difficulty Reaching Behind the Back

Reaching behind the body is often one of the most difficult movements with frozen shoulder. Tasks such as fastening clothing, reaching into a back pocket, putting on a belt, or washing the lower back may become challenging. This movement can remain limited even after overhead movement begins improving. Rather than forcing the arm aggressively, treatment usually progresses mobility based on the person’s symptoms and stage of recovery.

The Stages of Frozen Shoulder

Frozen shoulder is commonly described as moving through three general stages. The amount of time spent in each stage varies from person to person.

Freezing Stage

During the freezing stage, pain gradually increases and shoulder movement begins to decrease.

Everyday movements may become more uncomfortable, and night pain can be significant. The shoulder may feel painful before the stiffness becomes severe.

Because the joint is often highly sensitive during this stage, frozen shoulder treatment usually avoids aggressive stretching that creates a major increase in pain.

Instead, the focus may be on maintaining comfortable movement, managing symptoms, and preventing unnecessary loss of function.

Frozen Stage

During the frozen stage, pain may begin to decrease while stiffness becomes the main problem.

The shoulder can feel stuck or blocked during reaching. Activities that require overhead movement, rotation, or reaching behind the body may remain difficult.

Treatment during this stage may gradually place more emphasis on restoring mobility and improving shoulder strength.

Thawing Stage

During the thawing stage, shoulder movement gradually begins to return.

Daily activities become easier as stiffness improves. Strength may still need to be rebuilt after months of limited use.

Physical therapy can help guide the return of movement while strengthening the shoulder and preparing the arm for normal daily activities.

Causes of Frozen Shoulder

The exact cause of frozen shoulder is not always known. Some people develop the condition without an injury or another obvious shoulder problem. In other cases, frozen shoulder begins after surgery, trauma, or a period when the shoulder has been kept relatively still. A person recovering from a fracture, rotator cuff injury, or surgery may reduce shoulder movement because of pain or medical restrictions. This period of limited movement can increase the risk of developing significant stiffness. However, not everyone who rests a shoulder develops frozen shoulder. Several biological and medical factors may contribute.

Frozen Shoulder After Surgery

Shoulder or upper-body surgery can sometimes be followed by significant stiffness. Early movement may be limited because the tissues need time to heal. Pain can also cause a person to protect the arm more than necessary. Rehabilitation after surgery must balance protecting the healing tissue with gradually restoring movement. If frozen shoulder develops, the treatment plan must also consider the original surgery and any restrictions from the surgeon.

Frozen Shoulder After an Injury

A fracture, rotator cuff injury, fall, or other painful shoulder problem can lead to reduced arm movement. When the shoulder remains guarded for a long time, stiffness may gradually increase. Diabetes and Frozen Shoulder

The reason for this connection is not completely understood. Patients with diabetes may also have other health considerations that influence healing, pain, and exercise tolerance. Other Medical Risk Factors

Certain medical conditions have been associated with frozen shoulder. AAOS identifies conditions such as thyroid disorders, Parkinson’s disease, and cardiac disease among factors that may be associated with an increased risk. Having one of these conditions does not mean a person will develop frozen shoulder. However, medical history can provide useful information during the evaluation.

Frozen Shoulder and Rotator Cuff Problems

Frozen shoulder and rotator cuff problems can both cause shoulder pain and difficulty lifting the arm, but they are not the same condition. A rotator cuff injury may cause pain or weakness while the shoulder still maintains a reasonable amount of passive movement. With frozen shoulder, the joint itself becomes stiff. Even when the patient relaxes and someone else moves the arm, motion is still limited. Some people may have both conditions at the same time. A thorough examination can help determine which problem is creating the largest limitation.

Frozen Shoulder and Shoulder Arthritis

Shoulder arthritis can also cause pain and stiffness. Unlike frozen shoulder, arthritis involves changes in the joint surfaces and cartilage. X-rays may help identify arthritis when it is suspected. A person with arthritis may also experience grinding, joint swelling, or long-term changes in movement. Because the symptoms can overlap, medical evaluation may sometimes be needed before starting frozen shoulder treatment.

Effects on Daily Activities

Frozen shoulder can make simple tasks surprisingly difficult. Getting dressed may take longer because reaching into sleeves or fastening clothing becomes painful. Washing or styling hair may be hard because the arm cannot comfortably reach overhead. Driving may become challenging when the person cannot easily reach the steering wheel, fasten a seat belt, or turn the upper body. Housework, carrying groceries, reaching shelves, exercising, and caring for children may also become difficult.

A complete frozen shoulder treatment plan should focus on the activities the patient actually needs to perform rather than treating shoulder range of motion as the only goal.

Effects on Work

Frozen shoulder may interfere with both physical and desk-based jobs. People who perform overhead work, lifting, reaching, carrying, or repetitive arm movements may struggle with their normal duties. Desk workers can also become uncomfortable when the shoulder remains in one position for long periods. Temporary changes in workload, lifting height, reaching, or workstation position may help while movement gradually improves.

Physical therapy can also help prepare the shoulder for a safe return to more demanding work activities.

Physical Therapy Evaluation

Your first appointment begins with a discussion about when the shoulder symptoms started and how they have changed. Your therapist may ask about pain at night, difficulty reaching, previous injuries, surgery, diabetes, thyroid problems, and the activities you can no longer perform comfortably. During the physical examination, both active and passive shoulder movement may be measured. This helps determine whether the restriction is coming from weakness, pain, or true joint stiffness.

Strength of the shoulder and surrounding muscles may also be assessed. The shoulder blade, neck, elbow, and upper back can be examined because these areas work together during arm movement. Shoulder Rotation Exercises

Rotation is commonly limited with frozen shoulder, especially turning the arm outward. Gentle rotation exercises may help gradually restore this motion. Some exercises are completed with the elbow near the side of the body. Others may be added later in different arm positions. The amount of pressure used should match the shoulder’s current tolerance.

Shoulder-Blade Movement

The shoulder blade moves whenever the arm reaches overhead. When the shoulder joint becomes stiff, people often compensate by lifting or rotating the shoulder blade more than usual. Treatment may include exercises that improve awareness and control of the shoulder blade. The goal is not to stop the shoulder blade from moving. Shoulder Strengthening

Strengthening becomes increasingly important as pain decreases and movement improves. Months of limited arm use can cause weakness in the shoulder and surrounding muscles. Early exercises may use gentle resistance close to the body. Later, resistance bands, weights, lifting, and more functional movements can be introduced. Strengthening should help prepare the shoulder for reaching, carrying, work, exercise, and other normal activities.

Upper-Back Strengthening

The upper back helps support shoulder movement and posture. Exercises may include rowing movements, shoulder-blade control, and upper-back strengthening. Better strength around the shoulder blade can help the arm move more efficiently as shoulder motion returns. These exercises may also improve endurance for computer work, lifting, and prolonged activity.

Manual Therapy

Hands-on treatment may be used to improve shoulder mobility and reduce stiffness. Physical therapists may use gentle joint movement, assisted stretching, or soft-tissue techniques depending on the stage of frozen shoulder. Returning to Exercise

Patients who regularly lift weights, swim, play sports, or complete fitness classes may need a gradual return. The shoulder should regain enough movement and strength for the activity before adding heavy resistance or demanding overhead exercises. Early training may focus on movements below shoulder height. As mobility improves, the patient can gradually return to overhead strengthening, pushing, pulling, and sport-specific activities.

Injections and Medical Treatment

Some patients receive corticosteroid injections to help reduce pain and improve the ability to participate in stretching and physical therapy. AAOS notes that steroid injections into the shoulder joint may help reduce pain and improve mobility, particularly earlier in the course of the condition. The decision to use an injection should be made with a medical provider. An injection does not automatically restore full movement. Rehabilitation may still be needed to address stiffness and weakness.

Hydrodilatation

Hydrodilatation, sometimes called joint distension, is another treatment that may be considered when symptoms remain difficult to manage. During this procedure, fluid is injected into the shoulder capsule to stretch the tight tissue. AAOS lists hydrodilatation among nonsurgical options for frozen shoulder. Physical therapy may continue afterward to help maintain and improve the motion gained. Whether this procedure is appropriate depends on the individual patient and should be discussed with a medical provider.

Surgery for Frozen Shoulder

Most people with frozen shoulder improve without surgery. When symptoms remain severe despite a long period of nonsurgical care, surgery may be considered. Procedures may include manipulation under anesthesia or arthroscopic release of the tight shoulder capsule. Physical therapy is usually an important part of recovery afterward because shoulder movement must be maintained while strength gradually returns.

Recovery Time

Frozen shoulder is known for recovering slowly. Each stage may last for several months, and MedlinePlus notes that complete healing can take as long as 18 to 24 months. Some people improve sooner, while others need considerably more time. Progress may include better sleep, easier dressing, greater overhead reach, less pain, improved rotation, and the ability to complete daily tasks with less compensation. Persistent Shoulder Stiffness

Some people continue to have mild stiffness even after the most painful phase has passed. The shoulder may improve enough for normal daily activities but still feel different from the opposite side. Continued mobility and strengthening may help restore additional function. If progress suddenly stops or symptoms become significantly worse, the shoulder should be reassessed to make sure another condition is not contributing.

Preventing Loss of Strength During Recovery

It can be tempting to stop using the arm completely when movement hurts. However, prolonged inactivity may lead to weakness in the shoulder, arm, and upper back. Your therapist can identify movements that are safe to continue while avoiding activities that create a major flare-up. Maintaining general physical activity may also help preserve overall strength and endurance during a lengthy recovery.

Medical Warning Signs

Seek prompt medical care after a fall or accident when shoulder pain is severe, the arm appears deformed, or you cannot move the arm. Sudden weakness following an injury may indicate a significant tendon or nerve problem. New numbness, tingling, or loss of feeling in the arm or hand should also be evaluated. A red, hot, swollen shoulder accompanied by fever may indicate infection and requires medical care.

Chest pain, shortness of breath, sweating, nausea, or pain spreading from the chest into the shoulder or arm requires emergency evaluation because shoulder discomfort can sometimes be related to heart problems. Pain that continues to worsen, unexplained weight loss, or severe pain that does not change with shoulder movement should also be discussed with a medical provider.
