Understanding Achilles Tendonitis

Achilles tendonitis describes pain and irritation involving the tendon at the back of the ankle. The condition is also commonly called Achilles tendinopathy. The term tendonitis suggests inflammation. However, symptoms that continue for a longer period may also involve changes within the tendon rather than inflammation alone. For this reason, treatment usually focuses on gradually improving the tendon’s ability to manage force instead of relying only on rest.

Achilles tendon problems often develop slowly. A person may first notice mild stiffness in the morning or soreness after activity. Over time, the area may become more painful during walking, running, stairs, or exercise. The tendon may also feel thick, tender, or swollen. Some people notice a small bump or firm area along the tendon or near the heel.

Achilles Tendon Location and Function

The Achilles tendon is located at the back of the lower leg. It begins where the calf muscles narrow and continues down to the heel bone. Its main job is to transfer force from the calf muscles into the foot. This allows the heel to rise and the body to move forward. The tendon works when standing on the toes, climbing stairs, walking uphill, running, jumping, and pushing off the ground.

Because the Achilles tendon manages repeated force throughout the day, a sudden increase in activity can overload it. The tendon may not have enough time to adapt before the next workout or activity.

Common Achilles Tendonitis Symptoms

Pain is usually felt at the back of the ankle or heel. The area may be tender when touched, especially along the tendon or where it attaches to the heel bone. Morning stiffness is common. The first few steps after getting out of bed may feel painful or tight. Symptoms may ease after moving for several minutes and then return later in the day.

Pain may increase during walking, running, jumping, or climbing stairs. Standing on the toes or performing a heel raise may also become uncomfortable. The tendon may feel warm, swollen, or thicker than the tendon on the opposite side. Shoes that press against the back of the heel can sometimes increase irritation. As the condition becomes more irritated, pain may begin earlier during activity and remain longer afterward. In more advanced cases, normal walking and standing may also become difficult.

Midportion Achilles Tendonitis

Midportion Achilles tendonitis affects the section of the tendon located above the heel bone. Pain is often felt several inches above the heel. The area may feel tender, thick, or swollen. Symptoms commonly increase during running, jumping, hills, stairs, or repeated heel raises. Morning stiffness may also be noticeable. Some people find that symptoms improve as they warm up. However, pain may return after exercise or later that day.

Insertional Achilles Tendonitis

Insertional Achilles tendonitis affects the area where the tendon attaches to the heel bone. Pain is usually felt directly at the back of the heel. Shoes may rub against the area and make it more sensitive. Deep ankle bending can compress the tendon against the heel bone. For this reason, uphill walking, deep squats, stairs, and calf stretches performed with the heel below a step may increase symptoms. A firm bump may form near the back of the heel. This does not always mean surgery is required, but it may affect shoe comfort and exercise selection.

The difference between midportion and insertional symptoms matters because some exercises need to be adjusted based on where the tendon is painful.

Common Causes of Achilles Tendonitis

Achilles tendonitis often develops after a change in activity. Running farther, moving faster, adding hills, or exercising more days each week can increase the load placed on the tendon. Returning to sports after time away may also cause problems. Even when the activity feels familiar, the tendon may no longer have the same strength or endurance it had before the break. Jumping sports can place a high demand on the tendon because the calf and Achilles must repeatedly absorb and produce force. Basketball, volleyball, tennis, soccer, and pickleball may contribute when activity increases too quickly.

Long periods of walking or standing may also cause symptoms, especially when a person changes jobs, travels, begins a new fitness routine, or suddenly becomes more active. The condition is not always caused by one movement or one problem. Training load, strength, recovery, footwear, ankle mobility, and overall health may all contribute.

Training Changes and Tendon Overload

Tendons become stronger when they are given the right amount of challenge and enough time to recover. Problems may begin when activity increases faster than the tendon can adapt. For example, a runner may tolerate level running but develop pain after adding hills and speed workouts during the same week. Workout intensity is only one part of tendon load. More weekly sessions, fewer rest days, and longer periods on the feet can also increase stress.

A physical therapist can help identify which part of the training routine may be causing the overload. Temporary changes can then be made without removing all activity.

Calf Strength and Endurance

The calf muscles work directly with the Achilles tendon. Weakness or reduced endurance may make it harder for the lower leg to manage repeated walking, running, and jumping. A person may be able to complete several heel raises but struggle as the repetitions continue. This drop in endurance can become important during longer runs, sports practices, or work shifts.

Both major calf muscles need to be considered. One works more with the knee straight, while the other becomes more active when the knee is bent. Rehabilitation often includes several types of calf exercises, so the tendon is prepared for different positions and activities.

Ankle Mobility and Lower-Leg Movement

Limited ankle movement may change how the foot and leg move during walking, stairs, squatting, and running. If the ankle does not move forward comfortably over the foot, the body may compensate through the foot, knee, hip, or opposite leg. Too much stretching is not always helpful, especially when the tendon is painful where it attaches to the heel. Deep ankle bending can compress an irritated insertional tendon. Your therapist will determine whether mobility work is needed and which movements are safe for the location of your symptoms.

Footwear and Heel Pressure

Shoes that press against the back of the heel may increase pain, especially with insertional Achilles tendonitis. A stiff heel counter, worn cushioning, or a sudden change in shoe style may also affect comfort. Some people feel better with a temporary heel lift because it reduces the amount of stretch and compression placed on the tendon. This should be based on the individual evaluation rather than treated as a permanent solution. Footwear can be helpful, but shoes alone do not restore tendon strength. Exercise and gradual activity progression are usually important parts of recovery.

Achilles Tendonitis and Achilles Tears

Achilles tendonitis and an Achilles tendon rupture are different conditions. Tendonitis usually develops gradually. Pain, stiffness, and swelling may increase over days, weeks, or months. A rupture often happens suddenly. A person may hear or feel a pop and describe the feeling as though someone kicked or struck the back of the ankle. Walking may become difficult, and the person may lose the ability to push off the foot or stand on the toes. Bruising and swelling can also develop. A suspected rupture requires prompt medical evaluation. Do not try to stretch or exercise through sudden severe pain and weakness.

Other Conditions That Can Resemble Achilles Tendonitis

Pain at the back of the heel can come from several different conditions. Retrocalcaneal bursitis affects a fluid-filled sac located near the Achilles tendon and heel bone. It may cause swelling, tenderness, and pain from shoe pressure. This condition can occur alongside Achilles tendon irritation. A heel bone stress injury may cause pain during walking and activity. Plantar fasciitis usually affects the bottom of the heel rather than the back, although symptoms can sometimes overlap.

Nerve irritation may cause burning, tingling, numbness, or unusual sensitivity. These symptoms are not typical of simple Achilles tendonitis. Pain in children and growing teenagers may involve the growth plate at the back of the heel rather than the tendon itself. An evaluation helps determine which structure is causing the symptoms and whether medical imaging is needed.

Effects on Walking and Daily Activities

Achilles tendon pain can affect nearly any activity that requires pushing through the foot. Walking may become slower, and the person may take shorter steps to avoid loading the painful side. Stairs can be difficult because the calf and tendon help lift and control the body. Walking uphill may also place more demand on the area. Standing from a low chair, carrying groceries, completing yard work, and spending a long day on the feet may increase symptoms. Athletes may have trouble running, jumping, cutting, or changing speed. Even when the pain feels mild, the tendon may not be prepared for repeated high-force movement.

Physical Therapy Evaluation

Your first appointment begins with a discussion about when the symptoms started, where the pain is located, and how the problem affects daily activities. Recent changes in exercise, sports, work, walking, running, and footwear will be reviewed. Your therapist may also ask about previous ankle injuries, medications, medical conditions, and past tendon problems. During the examination, the therapist may check the tendon for swelling, tenderness, thickening, and changes near the heel.

Ankle movement, calf strength, balance, walking, stairs, squatting, and single-leg control may also be assessed. Heel raises are commonly used to see how much force the calf and tendon can manage. Running, hopping, and jumping tests may be added later when symptoms and strength allow. If the findings suggest a rupture, fracture, or another serious condition, medical evaluation may be recommended.

Medical Imaging

Many Achilles tendonitis cases can be assessed without imaging. An X-ray may show changes around the heel bone, although it does not clearly show the tendon itself. Ultrasound or MRI may be used when a tendon tear is suspected, symptoms are not improving, or the diagnosis remains unclear. Imaging findings do not always match pain levels. Some people have tendon changes without pain, while others have significant symptoms without severe imaging findings. Treatment should consider both the examination and the activities the person needs to perform.

Achilles Tendonitis Treatment

Achilles tendonitis treatment often begins by adjusting the activities that are keeping the tendon irritated. This does not always mean complete rest. Instead, the goal is to reduce the most painful movements while maintaining safe activity. A personalized program then helps the tendon gradually rebuild strength and tolerance. Physical therapy may include education, pain management, mobility work, calf strengthening, balance exercises, walking or running changes, and a gradual return to activity.

Activity Changes During Recovery

Running, jumping, hills, or repeated stairs may need to be reduced during the painful stage. The amount of change depends on how the tendon responds. Mild soreness that settles quickly may be managed differently from pain that causes limping or remains worse the following morning. Lower-impact exercise may help maintain fitness. Cycling, swimming, or another comfortable activity may be used when appropriate. Complete rest for a long period can lead to more weakness. The tendon usually needs gradual loading to rebuild its ability to manage force.

Pain and Swelling Management

Ice may provide short-term relief after activity. A cloth should be placed between the ice and skin. A temporary heel lift or taping technique may reduce strain and improve comfort for some patients. Shoes that rub against the heel may need to be changed temporarily. Medication questions should be discussed with a physician or pharmacist, especially when other health conditions or medications are involved. Pain-relief methods can make movement more comfortable, but they should be combined with exercises that rebuild tendon strength.

Early Calf Exercises

Early strengthening may begin with gentle calf contractions that create tension without a large amount of movement. A patient may press the front of the foot into the floor or hold a supported heel raise position. These exercises can help reduce sensitivity while beginning to restore muscle and tendon function. The amount of pressure should remain manageable. Sharp pain or a major increase in symptoms afterward may mean the exercise needs to be adjusted.

Heel Raise Strengthening

Heel raises are a major part of many Achilles rehabilitation programs. Treatment may begin with both feet on the floor. As strength improves, more weight can be shifted toward the painful side. The program may progress to single-leg heel raises, added resistance, slower lowering, or heavier calf training. Exercises may be completed with the knee straight and bent to strengthen different portions of the calf.

For insertional Achilles symptoms, the heel may need to stay level with the floor rather than dropping below a step. This reduces compression where the tendon attaches to the heel.

Progressive Tendon Loading

The Achilles tendon must tolerate more than basic walking. It may need to manage running, jumping, quick direction changes, and repeated force. Strengthening should become gradually harder as symptoms improve. Later exercises may include heavier calf raises, faster heel raises, loaded walking, step work, and single-leg movements. Athletes may progress to hopping, jumping, landing, sprinting, and sport-specific drills. Progress should be based on the tendon’s response during exercise and over the next day, not only on how it feels during the session.

Hip, Knee, and Foot Strengthening

The calf and Achilles work as part of the entire leg. Hip weakness may affect balance and control during walking, running, and landing. Foot weakness may change how the body pushes from the ground. Treatment may include squats, step-ups, lunges, bridges, foot exercises, and single-leg strengthening. These movements help prepare the whole leg for daily activities and sports rather than focusing only on the painful area.

Balance and Single-Leg Control

Walking and running require the body to repeatedly balance on one leg. Achilles pain can reduce confidence and cause the person to shift weight away from the injured side. Balance training may begin with supported single-leg standing. Reaching, stepping, uneven surfaces, and movement challenges can be added later. Athletes may eventually practice landing and changing direction while maintaining good control.

Walking and Stair Training

A painful tendon may cause limping or reduced push-off during walking. Your therapist may help restore an even step pattern and gradually increase walking distance. Stair training may begin with a smaller step or added support. As strength improves, the patient can practice controlling the body while stepping up and down. The goal is to return to normal movement without sharply increasing symptoms.

Running Assessments

Runners may benefit from an assessment of training habits and movement. Your therapist may review mileage, speed, hills, surfaces, shoes, and recovery days. Running form may also be observed. Stride length, step rate, foot contact, and control of the hip and knee may be considered. Small changes may help some runners, but the goal is not to force everyone into the same running style. Any adjustment should be introduced gradually while the tendon continues to build strength.

Gradual Return to Running

Running usually begins after walking is comfortable and calf strength has improved. A return-to-run program may alternate short jogging periods with walking. Distance is generally increased before hills, speed, or longer runs. The tendon’s response should be checked during the run, later that day, and the following morning. A small amount of manageable soreness may sometimes occur during rehabilitation. However, pain that continues to build, causes limping, or remains clearly worse the next day may mean the progression was too fast.

Return to Sports

Sports place different demands on the Achilles tendon. A basketball player may need repeated jumping and landing. A soccer player may need sprinting and quick changes in direction. A pickleball or tennis player may need short bursts and side-to-side movement. Rehabilitation should prepare the tendon for these specific tasks. The final stage may include hopping, jumping, cutting, acceleration, and repeated drills while tired. Returning only because the pain has improved may not be enough. Strength, power, confidence, and activity tolerance should also be restored.

Hands-On Treatment

Hands-on care may be used to improve ankle movement or reduce tightness in nearby muscles. Soft tissue work, gentle joint movement, or assisted mobility may help some patients move more comfortably. Deep pressure directly over a painful tendon is not always appropriate. Manual therapy may provide short-term relief but strengthening and activity progression are usually needed for lasting improvement.

Home Exercise Program

Your home program may include calf strengthening, mobility, balance, and activity guidance. Exercises should match the location of the tendon pain and the stage of recovery. The program may change as the tendon becomes stronger. A person who is beginning with supported heel raises will eventually need more challenging exercises before returning to running or sports. Consistency is important. A short program completed regularly is often more useful than a long plan that is difficult to follow.

Achilles Tendon Injections and Medication Considerations

Medication and injection decisions should be discussed with a qualified medical provider. Corticosteroid injections near the Achilles tendon require caution because they have been linked to a greater risk of tendon rupture. Certain medications, including fluoroquinolone antibiotics, have also been associated with tendon irritation and rupture. Patients who develop new Achilles pain while taking one of these medications should contact the prescribing provider promptly. Do not stop a prescribed medication without medical guidance.

Recovery Time

Achilles tendonitis often takes time to improve because tendons adapt more slowly than muscles. Mild symptoms may begin improving within several weeks. More persistent cases may require several months of progressive strengthening and activity changes. The length of recovery depends on how long the symptoms have been present, the location of the pain, overall health, activity demands, and consistency with rehabilitation.

Progress may include less morning stiffness, easier walking, improved heel-raise strength, and the ability to complete longer activities without a major increase in pain. Recovery is not always a straight line. Temporary soreness may occur when a new exercise or activity is introduced.

Preventing Achilles Tendonitis From Returning

A gradual training plan can reduce the chance of symptoms returning. Mileage, speed, hills, jumping, and workout frequency should not all increase at the same time. Calf strengthening may need to continue after pain improves. The tendon must remain prepared for the force of walking, running, and sports. Rest and recovery days are also important. Hard sessions placed too close together may not give the tendon enough time to adapt.

Shoes should be comfortable and appropriate for the activity. Heavily worn shoes may need to be replaced. A warm-up can include easy walking, light jogging, and gradual movement before faster or more demanding activity begins.

Medical Warning Signs

Prompt medical care is needed after a sudden pop, severe pain, or the feeling of being struck at the back of the ankle. Difficulty walking, pushing from the foot, or standing on the toes may indicate a serious tear. Rapid swelling, bruising, or a visible change in the tendon should also be evaluated. Seek urgent care when the foot becomes cold, pale, blue, or numb. Calf swelling, warmth, redness, chest pain, or trouble breathing may indicate a blood clot or another serious condition and requires immediate medical attention.

Pain that continues to worsen despite reduced activity should also be checked.
