Muscle Strains


A muscle strain can happen in an instant. You sprint for a ball, lift something heavy, slip on uneven ground, or make a quick movement and suddenly feel a pull or sharp pain. Other strains develop more gradually. A muscle may become sore after doing more activity than usual, returning to exercise after time off, or repeatedly performing the same movement.
Whether the injury happened during a sport, at work, in the gym, or during everyday life, muscle strain physical therapy can help you safely rebuild strength, mobility, and confidence. What Is a Muscle Strain?
A muscle strain is an injury involving a muscle or the tissue connecting muscle to tendon. People often call this a pulled muscle. Strains can occur when muscle fibers are stretched beyond what they can currently tolerate or when the muscle is exposed to a sudden or excessive force. The injury can range from a mild strain involving a small number of muscle fibers to a more significant tear. Muscle strains can happen almost anywhere, but they are especially common in the legs and back.
What Does a Pulled Muscle Feel Like?
Symptoms depend on the muscle involved and severity of the injury. You may experience a sudden pulling sensation, localized pain, tenderness, stiffness, swelling, weakness, or difficulty using the injured area. Some people notice pain only when they stretch or contract the muscle. More significant strains can cause bruising and noticeable loss of strength. If the injury is severe, you may have difficulty walking, lifting, running, or performing other normal activities.
What Causes Muscle Strains?
Muscles are designed to tolerate force. Problems occur when the demand placed on a muscle exceeds what it can currently handle. A strain may happen during a sudden acceleration, change of direction, heavy lift, awkward movement, fall, or overstretching motion. Fatigue can also play a role. As muscles become tired, their ability to manage repeated force may change. This is one reason strains sometimes happen near the end of a game, workout, workday, or long period of physical activity.
Acute Muscle Strains
An acute muscle strain happens suddenly. You may know the exact moment it occurred. Perhaps you felt your hamstring pull while sprinting or experienced sharp pain while lifting something from the floor. Symptoms can develop immediately and may include pain, tenderness, weakness, swelling, or bruising. The first priority is determining the severity of the injury and whether additional medical evaluation is necessary.
Muscle Strains From Overuse
Not every strain happens during one dramatic movement. Some muscle problems develop after repeated activity or a sudden increase in workload. Maybe you started running again after several months away. Perhaps you spent an entire weekend doing yardwork after being relatively inactive. Your muscles may simply have been asked to tolerate more than they were prepared for. Physical therapy can help rebuild the capacity needed for those activities.
Grades of Muscle Strains
Muscle strains are often described using three general grades.
A Grade 1 strain is relatively mild. A small amount of muscle tissue is injured, but you can usually still use the muscle.
A Grade 2 strain involves more significant muscle fiber damage. Pain, weakness, swelling, or bruising may be more noticeable.
A Grade 3 strain is a complete or near-complete tear and can cause substantial loss of function.
The grade alone does not determine your entire recovery plan. The specific muscle, location of the injury, your activity demands, and other factors also matter.
Common Areas for Muscle Strains
Muscle strains can happen throughout the body. The hamstrings, quadriceps, calves, groin, hip muscles, back, shoulders, and arms are commonly affected. Athletes may be more likely to strain muscles during sprinting, jumping, throwing, cutting, or other explosive movements. Muscle strains can also occur during everyday tasks such as lifting, carrying, climbing, gardening, or home projects.
Hamstring Strains
The hamstrings run along the back of the thigh. These muscles are especially important during running and sprinting. A hamstring strain may cause sudden pain along the back of the thigh, tenderness, weakness, or difficulty walking and running. More significant injuries may cause bruising. Rehabilitation usually progresses from restoring comfortable movement to rebuilding strength and eventually preparing the hamstring for faster and more demanding activity.
Quadriceps Strains
The quadriceps are the large muscles along the front of your thigh. They help straighten the knee and play an important role during walking, running, jumping, squatting, and climbing stairs. A quadriceps strain can make bending the knee uncomfortable and may cause pain when walking or contracting the thigh. Physical therapy can help gradually restore mobility and strength before progressing toward higher-level activities.
Calf Strains
A calf strain can happen while running, jumping, pushing off the ground, or quickly changing direction. Some people describe feeling as though something struck the back of their lower leg. Walking may initially be uncomfortable, especially when pushing off through the toes. Rehabilitation may include restoring ankle mobility and progressively strengthening the calf before returning to running and jumping. Sudden calf pain and swelling can occasionally have causes other than a muscle strain, so unexplained symptoms should be medically evaluated.
Groin Strains
Groin strains commonly involve the adductor muscles along the inner thigh. They can occur during quick changes of direction, kicking, skating, running, or movements that rapidly stretch the inner thigh. Pain may be felt near the groin or along the inside of the thigh. Rehabilitation typically focuses on gradually rebuilding adductor strength along with hip and trunk control.
Hip Muscle Strains
Several muscles surrounding the hip can become strained. Symptoms may occur along the front, side, or back of the hip depending on the muscle involved. Your therapist can evaluate hip strength, mobility, and movement to determine which tissues appear to be contributing. Treatment can then progress toward the activities that caused the problem in the first place.
Back Muscle Strains
Muscles surrounding the spine can become irritated during lifting, twisting, sports, repetitive work, or sudden movements. A back strain may cause localized soreness, stiffness, or muscle spasms. Most episodes of uncomplicated muscular back pain improve with appropriate movement and gradual return to activity. However, back pain accompanied by significant neurological or other concerning symptoms requires additional evaluation.
Shoulder and Upper-Body Muscle Strains
Muscle strains can also affect the shoulder, chest, upper back, and arms. They may occur during weightlifting, throwing, repetitive work, home projects, or sudden movements. Because several muscles and tendons work together around the shoulder, it can sometimes be difficult to tell whether symptoms are coming from a muscle strain or another injury. A physical therapy evaluation can help identify the movements and tissues involved.
Muscle Strains From Sports
Sports place high demands on muscles. Running, cutting, jumping, throwing, kicking, swinging, and sudden changes of direction all require muscles to produce and absorb force quickly. A sports rehabilitation program should eventually prepare the injured muscle for those same demands. Simply being able to walk without pain does not necessarily mean an athlete is ready to sprint or compete.
Muscle Strains From Lifting
Lifting injuries can happen at the gym, at work, or around the house. The problem is not always that you used “bad form.” Sometimes the load was simply heavier than your body was prepared to handle at that moment. Other times fatigue, speed, repetition, or an unfamiliar movement contributed. Physical therapy can help rebuild strength and practice the lifting tasks you need to perform.
Muscle Strains at Work
Physical jobs may require repeated lifting, carrying, pushing, pulling, climbing, reaching, or working in difficult positions. A muscle strain can interfere with your ability to safely complete these tasks. Rehabilitation should consider your actual job demands. Your therapist can gradually progress exercises toward the loads, positions, and movements you need at work.
How Are Muscle Strains Diagnosed?
Many muscle strains can be identified through your medical history and physical examination. Your provider may ask how the injury happened, where you feel pain, and which activities make symptoms worse. A physical examination may include movement, strength, flexibility, tenderness, and functional testing. Imaging is not always necessary. More severe injuries or situations where the diagnosis is unclear may require ultrasound, MRI, X-ray, or another medical test.
Muscle Strain or Muscle Soreness?
Muscle soreness after unfamiliar exercise does not necessarily mean you strained something. Delayed onset muscle soreness, commonly called DOMS, often develops after exercise your body is not accustomed to. It usually becomes noticeable several hours later rather than causing a sudden sharp pain during the activity. A strain is more likely to involve localized pain, weakness, tenderness, or a recognizable injury event. If you are unsure, an evaluation can help determine what may be happening.
Muscle Strain or Tendon Injury?
Muscles transition into tendons, which connect muscle to bone. This means pain around these areas can sometimes be difficult to identify on your own. Tendon problems may behave differently from an acute muscle strain and can require a different rehabilitation approach. Your therapist will consider the location of your symptoms, how the injury happened, strength testing, and which movements reproduce your pain.
Muscle Strain or Tear?
A strain can technically involve tearing of muscle fibers. The important question is how much tissue is involved. A mild strain may involve only a small number of fibers. A severe strain can involve a large portion of the muscle or a complete rupture. Significant weakness, extensive bruising, a visible change in the muscle, or inability to use the injured area warrants medical evaluation.
Should You Stretch a Pulled Muscle?
Stretching may feel like the obvious thing to do when a muscle feels tight. But aggressively stretching a newly injured muscle is not always helpful. Early rehabilitation should respect the healing tissue. Gentle mobility may be appropriate, but forcing the muscle into a painful stretch can unnecessarily irritate the area. As healing progresses, your therapist can determine whether flexibility work should be added and how much is appropriate.
Should You Massage a Muscle Strain?
Massage and other hands-on techniques may sometimes help with comfort or surrounding muscle tension. However, aggressive pressure directly over a fresh muscle injury may not be appropriate. Manual therapy should be selected based on the stage and severity of the injury. More importantly, massage does not replace the progressive strengthening needed to prepare the muscle for normal activity again.
Should You Rest a Muscle Strain?
Some activity modification is often helpful immediately after an injury. That does not necessarily mean complete rest until every symptom disappears. Too much inactivity can contribute to stiffness, weakness, and loss of conditioning. As the injury begins settling, gradually reintroducing appropriate movement can help prepare the tissue for increasing demands. Your therapist can help determine what you can safely continue doing while you recover.
Ice or Heat for a Muscle Strain?
Ice or heat may provide temporary comfort for some people, but neither replaces rehabilitation. Early after an injury, some people prefer cold because it temporarily reduces discomfort. Others find warmth more comfortable once the initial injury has settled. The more important part of recovery is gradually restoring movement and muscle capacity. Use either option for comfort rather than assuming it is repairing the injured tissue.
Can Physical Therapy Help a Muscle Strain?
Yes. Muscle strain physical therapy can help guide recovery from the early stages of an injury through return to normal activity. Treatment may begin with gentle movement and exercises that place manageable amounts of load through the injured muscle. As symptoms improve, the muscle needs progressively greater challenges. Strengthening can then advance toward the movements, speeds, and forces required for your job, sport, exercise routine, or everyday activities.
Strengthening After a Muscle Strain
Strengthening is one of the most important parts of rehabilitation. An injured muscle needs time to heal, but it also needs to regain its ability to produce and absorb force. Exercises usually begin at a manageable level. Resistance, range of motion, speed, and complexity can gradually increase. Later stages of rehabilitation may include heavier resistance, faster movements, jumping, running, or sport-specific exercises when appropriate.
Returning to Running After a Muscle Strain
Being able to walk without pain does not automatically mean you are ready to run. Running places much greater demands on muscles. Your therapist may assess walking, strength, single-leg control, hopping, and other movements before progressing your running program. Return to running may begin with shorter intervals at an easier pace. Distance and speed can then gradually increase.
Returning to Sports
Athletes need more than basic strength before returning to competition. Your rehabilitation may eventually include sprinting, jumping, cutting, throwing, kicking, or other sport-specific movements. The injured muscle needs to tolerate those forces before you return to full competition. Your therapist can gradually expose the muscle to increasing demands and monitor how it responds.
Returning to the Gym
A muscle strain does not necessarily mean you need to stop exercising completely. You may be able to train around the injury while it heals. Your therapist can help determine which exercises are appropriate and how to modify load, range of motion, or intensity. As your strength improves, you can gradually work toward your normal training routine.
How Long Does a Muscle Strain Take to Heal?
Recovery time varies significantly. A mild strain may improve relatively quickly, while a larger injury can take considerably longer. The muscle involved also matters. Your age, general health, activity demands, previous injuries, and severity of tissue damage can all influence recovery. Rather than relying entirely on a calendar, your therapist can monitor strength, movement, symptoms, and functional performance to determine when you are ready to progress.
Why Does My Muscle Still Feel Tight?
A muscle can continue feeling tight even after the initial injury begins healing. That sensation does not always mean the muscle physically needs more stretching. Weakness, sensitivity, protective muscle guarding, or reduced confidence using the area can all contribute to feelings of tightness. Your therapist can determine whether mobility, strengthening, gradual exposure, or another approach is most appropriate.
Can Muscle Strains Come Back?
Yes. Some muscle strains have a meaningful risk of reinjury, particularly if someone returns to demanding activity before regaining adequate strength and capacity. Feeling better is important, but symptoms are only one part of determining readiness. Your therapist may also assess strength, endurance, power, movement, and sport-specific performance. A good rehabilitation program prepares you for what you are actually returning to.
Preventing Future Muscle Strains
Not every injury can be prevented. However, improving your body’s ability to tolerate the demands you place on it may reduce your risk. Regular strength training, gradual increases in exercise volume, appropriate recovery, and preparing for the demands of your sport or job can all be helpful. If you are returning after an injury, completing the later stages of rehabilitation is particularly important.
What Happens During a Muscle Strain Evaluation?
Your therapist will begin by asking how the injury happened and what activities are currently difficult. The examination may include range of motion, strength, tenderness, flexibility, walking, balance, and functional movements. Athletes may eventually complete more demanding testing. Someone injured at work may need testing that reflects their job tasks. The goal is to determine what you need to safely return to your normal life.

When Should You Seek Medical Attention?
Some injuries need additional medical evaluation. Seek medical care if you experience severe pain, significant swelling or bruising, a visible deformity, a popping sensation followed by major loss of function, inability to bear weight, or substantial weakness. You should also seek evaluation if symptoms are getting worse instead of gradually improving. Sudden unexplained calf pain, warmth, redness, and swelling should be medically evaluated because symptoms of a blood clot can sometimes be mistaken for a muscle injury.
